Bachelors Level Internship/Practicum - Care Coordinator
Plains Area Mental Health Center is searching for interns to learn from our amazing Care Coordinators! This internship/practicum can be used for school credit, and we will work with your instructor to ensure you are meeting your requirements. We LOVE to hire Interns when they complete their collegiate career as well! Be sure to ask about future employment opportunities.
Applicants must be pursuing a degree in Social Work or Human Services to be considered.
You will be shadowing, learning, and working alongside a veteran Care Coordinator as they fulfill their responsibilities:
- Coordinate member enrollment/intake.
- Promote self-direction and skill development.
- Advocate for members.
- Conduct member needs assessments.
- Provide resources and referrals to community and social support services.
- Schedule appointments for members.
- Make and track referrals and appointments.
- Communicate with member’s Providers on interventions and goals.
- Communicate with the Interdisciplinary team on members to promote coordination of care.
- Promote member’s health and ensure all personal health goals are included in person-centered care plans.
- Engage potential caretakers as an alternative to emergency room or hospital care.
- Facilitate development of crisis plans.
- Monitor for potential crisis escalation or need for intervention.
- Conduct follow-up phone calls and face-to-face visits following an ER visit or hospital discharge.
- Identify and link to long-term care and home and community-based services.
- Identify community resources that will assist members to reduce barriers and achieve maximum health and success.
- Assist in transitional care services.
- Necessary data entry
