New Clients

Dear New Clients,

Thank you for choosing Plains Area Mental Health Center as your mental health provider. Plains Area Mental Health Center offers same-day access (Open Access). Open Access provides individuals the opportunity to receive a full diagnostic assessment with a therapist on the same day it is requested without a scheduling delay or waitlist for an initial appointment. The full diagnostic assessment completed during Open Access will be the determinant for what services are recommended for you. New Clients or individuals interested in starting services with Plains Area Mental Health Center are encouraged to come to one of our main office locations during our open access hours to start services. 

No clients will be denied access to services due to the inability to pay. We offer a discounted/sliding fee schedule available to those who qualify.

• Adult clients: Client must be present at intake.
• Dependent adult clients: For adult clients who are under a legal guardianship, legal guardian must be present at intake. please bring all legal paperwork (legal guardianship documents or medical power of attorney.) We will make copies for our records.
• Child clients: Legal guardian must be present at intake. If a child has been adopted or is in a custody situation, please bring all legal paperwork. We will make copies for our records.
• All clients: In order to provide continuity of care, we will need to receive records for all previous treatments. Please bring names and addresses of previous providers such as psychiatrists, therapists, substance abuse services, and mental health hospitals.

Referrals

If you are a provider interested in referring a client to Plains Area Mental Health, you can do so by filling out our online referral form. Plains Area Mental Health will contact the client you are referring to gather additional information and to encourage the client to walk into one of our main locations to complete an open-access intake. Please allow up to 48 working hours for us to contact the client and notify the client of our open access process. Thank you for your continued support of Plains Area Mental Health.
