First Time Logging in? 

You will need to contact your local Plains Area office to request portal access on your first time logging in. We will need to gather some demographic information and we will send you a secure link to log in for your first time. This link will have you establish a username and password, and the only time you will utilize that link. 

After you finish setting up your username/password, you will be abe to log in and access your account using the links on this page!

Create an Account

If you don't have an account yet, please follow the steps below. 

  1. Access the PatientPortal login page
  2. Select "Let's Get Started" under the "Need an account?" heading to the right
  3. Enter your email address and a password
  4. Select "Yes" that your provider gave you a pin
  5. Select "Myself" for whom you are registering
  6. Enter pin number
  7. Enter your date of birth
  8. Choose a security question and enter your answer
  9. Check the box to agree to the terms and conditions
  10. Click "Create My Account" to complete
